Million Dollar Round Table Foundation

North Shore Senior Center would like to extend a special "Thank You" to the Million Dollar Round Table (MDRT) Foundation for awarding a $5,000 grant to our Senior and Family Services/Community Care Program (CCP). 

Our Senior and Family Services Department provides services to seniors. Each senior is assigned a case manager. The case manager identifies needs, develops a care plan and, with the client’s agreement, arranges for services. We connect seniors to in-home and community-based services they need to stay healthy, remain independent, and live in their own homes for as long as possible.

North Shore Senior Center is the only agency authorized by the Illinois Department on Aging to provide CCP in New Trier, Maine, Northfield, Niles and Evanston Township. CCP was created to help seniors who were in danger of being placed in a Medicaid-funded nursing home. The program helps our most vulnerable seniors — those who are frail, physically or developmentally disabled, suffer from abuse, are economically disadvantaged, and/or have special needs. CCP clients have low-income or are living at the poverty level.

Community Care Program services include:

  • In-Home Care: A trained homemaker assists with personal care and household tasks. This includes cleaning, planning and preparing meals, laundry, shopping and running errands. They assist clients with their personal care tasks, such as dressing, bathing and grooming.
  • Adult Day Services: Care and supervision of clients outside their homes in a community location. Individuals can spend time engaged in various recreational and social activities. Transportation to the location is provided or arranged. Adult Day Services can also serve the needs of family members who may need respite from their caregiving responsibilities.
  • Emergency Home Response System: A 24-hour emergency communication link.

Last year, North Shore Senior Center served 6,362 CCP seniors by linking them to the programs and services they needed to live independently in their homes and their own communities for as long as possible. CCP costs North Shore Senior Center $10,000 daily. In order to support this program, the Center relies on private donors such as MDRT to support the funding gap from the State of Illinois budget situation.
